Thank you for your interest in serving on a Local Outreach team! We are excited you are pursuing to serve alongside your Jessup family and look forward to getting to know you better via your application.
Your Local Outreach application consists of three parts: 1) Application, 2) References and 3) Medical Form. Your application will be considered incomplete until all of the required forms are submitted. All forms are due before Monday, October 8. You cannot sign up for an interview if you are missing forms. Please submit all references and the Medical Form via email to email@example.com or bring them to the Student Life Office care of Mary Ann McMillan, Director of Outreach.

☐ New Participant Reference Form Two references are required: One from someone who can speak about your walk with Christ over the past two years (a pastor, small group leader, etc.) and one from a friend. The references cannot be filled out by anyone related to you.

PLEASE NOTE: A NON-REFUNDABLE deposit will be due once you are invited to and accept a team position on November 14, 2018. Please turn in a $100 deposit to Student Life once you receive your acceptance notification and would like to guarantee your spot on the team.
