Students are required to complete the student insurance process every year. All Traditional Undergraduate students carrying 6 units or more are automatically enrolled in the student health plan and charged accordingly. Students are required to complete the insurance process by filling out one of the following forms:
- Waiver Form(if you intend to waive out of the student health insurance policy)
- Appeal of Waiver Denial (if your waiver was denied)
- Enrollment Form(if you intend to enroll in the student health insurance policy)
Please make sure you log in and complete the process. We recommend doing this before you leave for summer, however, it MUST be completed by 8/8/14 if you are a student athlete and 9/5/14 for non-athlete students.
