In order to receive academic or residence accommodations, you will need to complete our applications process. Our intake forms include: (1) Disability Support Services Application, (2) FERPA release of information, and (3) student information form.
Additionally, you will need documentation of a disability from a licensed professional, such as a doctor, psychologist, or psychiatrist. Documentation must have the following in order to be accepted by Disability Support Services: be completed on the treating professionals letterhead, specifically name the type/nature of your disability, and reasoning why recommended accommodations may be beneficial.
Documentation may be physically mailed, emailed, or faxed. Traditionally email is the quickest and most secure method.

(1) In order to access some of these forms you will be prompted to login into google. Make sure to use your WJU Google account and not a personal email account.
(2) Many of the forms fields require answers, make sure to answer the areas that are required and to complete your form clicking the “submit” button.
