Required for Staff/Faculty:

Required for Students:
☐ WJU Mission and Doctrinal Statements Agreement Form
☐ SFG Leader Reference Form | Two references are required: One from someone who can speak intelligently about your walk with Christ over the past two years (a pastor, small group leader, etc.) and one from a peer. These references cannot be filled out by anyone related to you.
Required for Local Community Members:
☐ WJU Volunteer Agreement Form
☐ Background Check Disclosure and Authorization | This form will be emailed to you once you submit your volunteer agreement form
☐ WJU Mission and Doctrinal Statements Agreement Form
☐ SFG Leader Reference Form | One reference is required: preferably from your local church pastor. This reference cannot be filled out by anyone related to you.
